Foot and Ankle Appointments

Preparing for your visit your should bring:

• Your insurance card and one other form of ID

• A list of all medications that you are taking, or have recently taken

• Any part of your medical record, including x-rays, which may be helpful in providing your care

• Please fill out the following forms:

                  1)  Patient Registration Form

                  2)   History & Physical Form

• Cash, Checks, Credit Cards (Visa and Mastercard)
